Digital Credentialing Enrollment Procedure

Minimum computer requirements:
  • A Pentium Processor or equivalent computer
  • The current version of Internet Explorer web browser (IE 5.5 or better) and Adobe Acrobat Reader (versino 5.0 or better) are required to print the electronic forms.
  • Internet connectivity speed of at least 56K. Faster speeds increase performance.
  • An email account to which only you have access.
  • 1024x768 or higher monitor resolution for optimum viewing of forms.

Enrollment Steps:

1. Initiate the digital credentialing process and physician practice personnel enrollment by visiting http://www.mdkeybank.com/ . It is important to fill out an affidavit for each physician or professional being enrolled with the appropriate staff information. IMPORTANT: The physician retains the printed affidavit after the enrollment is complete. All physicians will be prompted for information from that form when they are initially activated by the Authentidate staff.

The following steps must be taken:
  • Type the information into the required fields on the initial page.
  • Press the 'Submit' button.
  • When the filled-out page is displayed, select 'Print' from the browser's 'File' menu.
  • Have the physician sign the form.
  • Fax the signed affidavit to the Inscrybe Help Desk (number on the form).

    If the form is not filled in on-line, but just printed. IMPORTANT: The information is written legibly. Failure to do so can lengthen the processing time.

2. A key person should be identified within the practice (office manager, lead nurse or administrator) to establish as SuperUser. IMPORTANT: This individual should be computer and web browser literate and capable of establishing identity profiles for all users within the physician practice. This person will be contacted by the Authentidate Inscrybe Call Center to confirm the physician practice enrollment and proper setup of form routing. Please inform office staff that the SuperUser will be receiving a phone call from the Authentidate Call Center.

3. The confirmation process will involve a short amount of the designated SuperUser's time to affirm the user group setup for proper form routing protocol. Knowledge of the current paper flow in the practice is required and the following criteria must be established:

  • Who is involved with the completion of Certificates of Medical Necessity and Plan of Care?
  • Do all participants or users have access to a web browser and email?
  • What path do the forms currently take within the practice?
  • Is there more than one physical location for the practice that forms are processed?
  • How many physicians are there within the practice that will participate in the electronic form system?
  • Have all of the physicians that wish to participate completed the Inscrybe MDKeyBank enrollment process?

4. The digital credentialing enrollment may also be initiated by contacting Healthcare Support with the pertinent information. Dial 1-877-467-2792 for Healthcare Support and provide the requested information to our healthcare support staff.
