Department of Veterans Affairs Care Coordination Home Telehealth

The Department of Veterans Affairs (VA) has contracted with Authentidate to be an authorized supplier  in the VA’s multi-year nationwide Care Coordination Home Telehealth (CCHT) program. CCHT was designed to improve the health of our nation’s veterans who have been diagnosed with chronic illnesses such as CHF, Diabetes or COPD, as well as other conditions. We have designed the optimum in-home telehealth patient monitoring solutions to improve overall care outcomes for patients with chronic illnesses beyond the hospital environment.


Authentidate’s VA contract includes Electronic House CallTM (EHC), the newest technology vital signs monitoring device in the VA program and our ExpressMD™ Interactive Voice Response (IVR) solution for mobile patients to enter vital signs and responses from any phone by voice response or key pad entry. Both solutions provide Care Coordinators with immediate access to their patients’ vital signs results and qualitative session question responses on the highly intuitive ExpressMD Web application. The ExpressMD Web application also integrates completely with the VA’s Computerized Patient Record System (CPRS).
